Adjunct Faculty Departmental Sponsoring Agreement

This form is for adjunct faculty to get permission to have library privileges. It needs to be signed by the department chair. If the patron fails to return their library materials, the department is responsible.

Visiting Scholar Departmental Sponsoring Agreement

This form is for scholars who are in the UND area for a limited time and working on a research project with a UND department. This agreement allows visiting scholars to checkout materials and use CFL resources while they are on campus. It does not cover access for scholars who are off campus or  working remotely with UND departments. The sponsoring department is responsible if a patron fails to return library materials.

Faculty Proxy Form

This form allows a student to checkout items or request ILL on behalf of a faculty member.

Course Reserves Form

This form is used by faculty to place items on course reserve.

Patron Registration Form

This form is for UND Alumni, UND Emeriti, and community members. It is used for registering as a CFL patron. This does not cover remote or off campus access to CFL resources for UND Alumni or community members. If you are a community member registering as a CFL patron, please use the UND ePayment site to pay the $50.00 annual fee.
