Faculty Proxy Policy
A student assistant may serve in the role of proxy when charging out materials for a faculty member. Staff may not proxy for another staff nor a student for another student. Exceptions may be allowed if the intent of the policy is not altered. Please contact the Access Services department if alternative arrangements are requested.
A Chester Fritz Library UND Faculty Proxy Agreement form identifying the student proxy must be signed by the faculty member and placed on file in the Access Services department along with a copy of the faculty ID. Proxies should retain a copy of the form for use when charging materials. Authorization may cover a period of time not to exceed one year from the date on the form, and must state the duration (term/semesters) covered, specifying beginning and ending dates. An electronic note to indicate such will be added to the faculty member’s library record.
When charging materials out of the Library for a faculty member, the proxy must provide his or her university ID along with (1) a copy of the faculty ID, and (2) a copy of the form from the faculty member.
Library users are responsible for fees associated with materials charged out in their names.
